DescriptionSummary:The Care Coordinator assists members/patients by providing support, education, and assistance in the prevention and/or maintenance of their health and wellness state. This role monitors members'/patients? treatment plans, educates them about resources available to support compliance with the plan of care, connects them with healthcare providers, and evaluates their progress.
